Southern Care Services Values

As a registered NDIS service provider, located in Harvey WA, Southern Care Services only take on participants on the NDIS who reside in Harvey, Yarloop, Cookernup, Brunswick or Waroona and surrounding areas. Having a strong focus on participants in these areas, with no other local NDIS approved service providers, ensures the participants NDIS dollar goes further. For example, reducing the cost of travel. Southern Care Services also only employ local support workers, cleaners, gardeners, with the hope of boosting the community economy and keep consistency in the workplace for participants. This will also reduce, with an ambition of avoiding any burnout of our workers through travel and changes to participants supports.

Southern Care Services have capacity to take on new participants who are on existing NDIS plans. Southern Care Services values choices and believes in the personal centred approach, ensuring that everyone is included and understood. Southern Care Services look at each participant in the context of their environment. Southern Care Services staff are innovative and creative and will tailor the supports that are essential to the participant and their family to safeguard the best outcome. Southern Care Services workers are highly trained and highly experienced and spend a significant amount of time understanding the dynamics of the participant requiring services and their families so that we can tailor the supports and make the NDIS dollar go as far as possible.

ASSISTANCE WITH DAILY TASKS IN GROUP

or shared living arrangements to help individuals to live as autonomously as possible.

Positive Behaviour Support

You will receive support from friendly and highly skilled Positive Behaviour Support Practitioners who will collaborate with you to enhance your quality of life, leading to increased happiness and improved independence.

ASSISTANCE WITH HOUSEHOLD TASKS

Such as meal preparation and delivery, house or yard maintenance, cleaning, and laundry.

ASSISTANCE WITH DAILY PERSONAL ACTIVITIES

to enable people to live as autonomously as possible – such as personal hygiene, dressing, medication assistance, financial Management, communication skills and lifestyle mentoring.

ASSISTANCE WITH TRAVEL AND TRANSPORT

provision of travel arrangements and support for people to access essential services.

ASSISTANCE WITH LIFE STAGE TRANSITIONS

including mentoring, peer support, individual skill development, and decision making to support people to make transitions on their own terms and in a way that meets their goals and aspirations.

ASSISTANCE TO ACCESS AND MAINTAIN EMPLOYMENT OR HIGHER EDUCATION

assistance and support with preparation for school leavers to find employment in either the supported or open job market, and in their transition through school towards further education.

THERAPEUTIC SUPPORTS

supports to build people’s physical and mental health, in order for them to move with optimal mobility and interact with enhanced confidence and interpersonal skills.

DEVELOPMENT OF DAILY LIVING AND LIFE SKILLS

including developmental-focused activities and training to enhance the person’s capacity to live as independent an everyday life as possible.

EARLY INTERVENTION SUPPORTS FOR EARLY CHILDHOOD

a mix of therapies to support children aged 0-6 with Autism Spectrum Disorder (ASD), developmental delay and/or speech delay.

GROUP AND CENTRE-BASED ACTIVITIES

such as social outings and group and social activities on Great Southern Care Services premises.

INNOVATIVE COMMUNITY PARTICIPATION

activities outside the scope of ‘Participation in the Community’.

PARTICIPATION IN THE COMMUNITY

including supported shopping, medical appointments, sporting and recreational events, social activities, visiting or making new friends, travel and transport and building confidence and social skills.

SUPPORT COORDINATION

A time limited support that focuses on addressing barriers and reducing complexity in the support environment, while assisting the person to connect with supports and build capacity and resilience.

NDIS PLANNING AND PLAN MANAGEMENT

Strengthening NDIS participants’ ability to undertake tasks associated with the Management of their supports. This includes building financial skills, organisational skills, and enhancing the participant’s ability to direct their supports and develop self-Management capabilities. Plan Management is the financial Management of the participant’s NDIS Plan and includes making payments to providers, expense claims processing, developing monthly statements for participants and claiming payment from the NDIA. Plan Managers also liaise with providers and participants to implement and manage the plan.
